If you have already contacted the store where you purchased your item and they have refused to help you, please describe your attempts to get the store to assist you with your problem on the form below. You **MUST** include the following specifics:

- – the date you contacted the store
- – the store’s response
- – as many other details as you can so that we can investigate the issue.
